How to Order Supplies

To order supplies for United American, please follow the instructions below.

1. Go to https://agentsupplygap.globelifeinc.com/SupplyForm/agentinfo/default.aspx.
2. Log in, and click the Products, Rates, Training button on the right side of the page under Agent Web Sites.
3. Select Products/Rates in the blue bar at the top of the page.
4. If you’re looking for an item that’s not an application, click the type of product (Medicare Supplement, Supplemental Health, Life, or Annuities) you need at the top of the page in red. Then click on a form to open the PDF. Download, print, or save the PDF. If you’re looking for an application, go to the next step.
5. For applications, click the state you want an application for.
6. Scroll down to the type of application you need.
7. Click the form number you need under the application column.
